happ coindoor problem registering coin input

Just got in my happs over/under, hooked it up to my ipac.   Couple of problems.

1) If I go to the ipac config "keyboard" tester and enter coins in the slot, it shows "5" key being registered, so appears to be working correctly.  I can drop 20 quarters in and it flashes "5" 20 times in that keyboard tester program.  Now when I go into mame (and mame is configured to register coin1 as "5") and I drop in quarters, sometimes it registers a "coin up" and sometimes it doesn't.  I drop in 3 and get 3 credits, then I drop in 4 more and it still stays at 3 credits, then I drop in 4 more and get 4 more credits, then drop several and it doesn't register those.

I've tried this multiple times.  So it seems to be hit/miss in mame, but seems to work correctly if I'm just monitoring the keyboard entry outside of mame.  I was wondering if maybe the "5" it's registering is too fast?  Like when I drop in the coin, it actually is getting a "5" (that's why it shows correctly in the keyboard tester) but for mame does it need to be a more "sustained" input? (i.e. as if I would hold the "5" key down longer)?  Any input?  And if that's the case, how can I fix that?

2) Unrelated to the above, but the metal clip that registers the coin pass through that is connected to the microswitch....on one of my coin inputs it sometimes gets stuck when a quarter passes and stays open.  I manually bent it a bit and it fixed it somewhat..just wondering if this is a normal tweak or some better fix to stop this from happening?

it's not related because it's in the other coin mech (not the one experiencing the behavior described in point 1).  So coin slot 2 has the sticking problem, but coin slot 1 and 2 have the "no registered coin-up" problem.

I have visually watched the coin fall through in problem 1 and it does not get stuck at all in that scenario.  It also works 100% outside of mame...so getting stuck isn't the problem (why would it just get stuck in mame and not outside of mame).

Is Coin switch 2 hooked up as Coin2 (so it registers a '6')?
Are the two coin switches sharing a ground wire? I'm wondering if perhaps when coin 1 gets stuck it's interrupting the circuit for coin 2...

coin 2 switch is set to coin 6 (quarter in slot 1 generates '5', quarter in slot 2 generates '6').  Both of those I've verified through that keyboard tester. 

They do not share the same ground wire (both wired to the seperate gnd input on my ipac)

I dont' think it's a matter of interrupting the circuit, because I can go directly from "keyboard test" to "mame test" and back and forth and it always registers correct '5' when outside of mame but only sometimes in mame.  I think if it was circuit related I would at least experience a few failures in the "keyboard test".  Also I'm not mixing the coin inputs..so in the above I'm only dropping coins in slot 1 during the keyboard and mame tests, so slot 2 would never get a chance to get stuck since I'm not dropping any coins during my test.

Seems if it was related to sticking or circuits, etc. that my keyboard test would fail at least occasionaly...but I can't recreate the failures in that keyboard test (it always shows a '5' for every quarter)...so that's why I was wondering if maybe the '5' input just isn't "strong" or "long" enough for mame to always recognize it?
