I'd recommend 2 coin slots/buttons and 2 start buttons so you're covered for all 2-player games and the 2-player versions of the 4-player games. YMMV.
For some 2-player games like Donkey Kong you may want both coin slot keystrokes mapped to the same MAME input since there is no "Coin 2" in the "Input (this Game)" menu.
Press Enter, Esc to clear the input, then press Enter, 5 (Coin 1), Enter, 6 (Coin 2) so MAME will recognize a token dropped in either coin slot.

If you add USB ports for gamepads, you can use them for P3 and P4, should a 3/4-player opportunity arise.
