Can anyone tell me the name of the cable I need here and where I can buy one? This is on the back of the CRT, which I presume is the video connection.

PBJ speaks truth. See next to it where it says "Input AC"? Any stripped power cable will work.

Yup, that's the power connector, not a video connector.
Looks like you have a Wells Gardner K7400 monitor there.
I believe this connector will fit:

Verify with the supplier first.
Or if you have the wires with pins on them, you can stick them in and tape 'em
