Screen suddenly scrambled - ArcadeVGA?

I posted in the Ultimarc boards as well, but thought I'd try here for exposure.  I've had my setup (Hanatrex monitor, AVGA, JPAC) working for a few months now with no problem. In fact I LOVE the ease at which the ArcadeVGA and JPac work together in a JAMMA cab.

My issue is that a couple days ago when I powered up the cab, the entire screen was scrambled. I attempted some easy tests (reboots, monitor v/h holds, etc.) but nothing seems to alleviate it. In fact, the green LED on my JPac never comes on - just the orange LED flashing VERY quickly.

What confuses me is that it just happened out of the blue, and I'm at a loss as to which component these symptoms seem to point to. When I replace the video card with a standard VGA card, the output to a PC monitor is fine.   It doesn't seem like the JPac would cause this - so could the ArcadeVGA card have gone bad in this short a time?
