MAME lay files: artwork in cocktail mode?

I'm planning to make my own bezels to use in MAME. So I've found Mr. Do's pages (http://mrdo.mameworld.info/mame_artwork_faq.php) but they point towards a non existant FAQ on mamedev.org.

Any help is appreciated. Does anyone have an example lay file to add a bezel to a cocktail view? I managed to get a bezel working for a normal game but so far no luck in switching to cocktail mode.

I found the missing pages using the wayback machine.
A more comprehensive guide is still welcomed  ;D
