Ipac problem

Hi,
I had my ipac running fine up to a few days ago.
I got a mate to redesign my panel as I only had 3 buttons per player now I have six.
I basically want 6 buttons per player, coin button at left side and pause button on right side.
I want the 2 top buttons to act as escape and enter with player 1 and 2 at the front.
I have tried everything to program the Ipac for these buttons but can't get it to work.
When I try to program the top left as escape the top right ends up getting programmed.
It also seems that some buttons are appearing in more than one location i.e P1 SW6 is P2 SW2 and two other buttons aswell.
I have both player 1 and 2 working with joystick controls and SW1, SW2 and SW3, Player 1+2 are fine so its only really the escape, enter, pause and coin that are causing me problems.
Sorry if I made this sound confusing.
Any help greatly appreciated.

ps Do I need to plug the keyboard into the ipac or pc?
I can't remember if I did when I first programmed it.
At the moment I just have the usb into the pc and into the pc ps2 slot on the Ipac board
