Looks like something Nintendo may have made exclusivly for a Childrens Hospital.

I know SEGA did the same kinda thing in the 90's with the Genesis. Then SEGA had the SEGA Channel piped into Childrens Hospitals all over the country!

Looks like someone probably has gotten this from an older hospital that probably was tucked away in some dark storage room. This was probably an auction item or the boss said "take it home, get it outta here!"

yeah could be real. Why not. Nintendo does lots of charity stuff. They also have put kiosk style game machines into McDonald's, and I've seen N64 based game systems in hotel rooms.
NO MORE!!

There have been things like that made third party as well over the years.  Looks real enough to me, whatever "real" has to mean here.  No matter what the criteria are it's still a TV, cart, and N64.

My oldest son used to get croop really bad when he was little.  We spent a few nights over the years in the emergency room where they had a setup like this.  I don't think he ever felt well enough to play it while we were there.  We were both really into the N64 at the time so we thought it was really cool that the hospital provided one.

Ya I realized that the game companies had done some charity work for hospitals, including game kiosks, I think it's great!
I just had never seen this particular type of unit and I might take a ride over to check it out, looks pretty cool!
I couldn't find one when I googled it, so naturally I asked here.
