Ah, I don't usually track those topics.
For MK2, you just need a 0.156" 9 position (NOT 8) Molex KK header on each end and 16 crimp contacts (buy 20, they're cheap and you'll screw some up). The IDC style are difficult to properly terminate without the tool, but the crimp style you can just mash/solder. I'm guessing you can find somebody to make you one of these (I can, if you wish) for a reasonable price if you don't want to make it yourself.
As you mention, it's straight through wired, but the middle position is not used (it's normally a keyway).
The parts can be bought at Digi-Key or Mouser (or other places). Parts are:
09-50-3091 - Housing w/ ramp (you can also use the style without the ramp, if you wish); you need 2 of these
08-50-0105 - Crimp contact; you need at least 16 of these.
I'd use AWG18 or AWG20 for this. AWG22 is a bit small given the amount of current it can pull.
