Mini-PAC not recognized by XP or Vista

I just tried my Ultimarc Mini-PAC for the first time and it is not being recognized by my MAME machine.  It is a Windows XP machine with service pack 3 on it and fully updated.  I plug it in and it tried to install it, but I get an error stating:

"USB Device is not recognized.  One of the USB devices attached to this computer has malfunctioned and Windows does not recognize it."

When I look under hardware devices there is a "Unkown Device" under USB Devices.  I have tried all the USB ports on my computer and get the same thing from all of them.

I also plugged it into my Vista machine and I get the same error.  Is there something I am missing?

You may try this from Ultimarc's site:

Problem: In USB mode, the I-PAC was not detected properly once before and now I can't get it out of this state.
You will need to remove it from Windows and let it re-detect. Go into Control Panel, System, Device Manager, Hardware. Open up the USB controller by clicking on the plus sign next to it. Under this heading will be displayed all the USB devices. Right click on all devices one by one except the controller itself and Root Hubs and select "uninstall". Now unplug and re-plug the I-PAC. It should be re-detected.

I have since tried the Mini-PAC on 3 other computers all with the same results.  They each come up as unknown device. 

I emailed Andy last night and he gave me some similar suggestions.  Try plugging it in without the harness.

Andy also asked me about what the LED did when I plugged it in.  I am not sure where the LED is on the board so if anyone can tell me that would be great.  I told him that I do not see anything flash, blink or stay lite.

Does anyone know if you use a PS2 to PS2 cable and plug the Mini-PAC into a keyboard connection if you need to provide power or does it get that through the PS2 connection?
