It's actually a Jedi or Empire one - same design, different stickers.

Here are the pics.  A few things:

Forgive the dust and cobwebs.  ;)

It's currently installed on a panel for my modular cab.  It'll come as pictured.  All you have to do is take it off that and put it on whatever you want.

It has the Empire stickers on it.

It's missing a couple of the dime-a-dozen screws for the metal cover.  It has one or two on it now and it stays on fine.

The hack is installed and works fine in MAME.  You will have to have the Dual Strike Sidewinder software to have the hack work.  I can provide that for you.

I also have a like new, complete in box, Dual Strike controller if you want to go your own hack in the future.  Make offers.
