ipac problem

I have a relatively new ipac4. It originally worked fine. Now, there seems to be a problem:

When I test the ipac with my buttons using Notepad, everything seems fine.

However, if I test in WinIPAC or PassMark, one button press many simultaneous presses (and none the key that the button is supposedly mapped to.

The ipac also doesn't work in Mame - because the button mappings don't appear to be working correctly, I can't use the buttons in Mame.

I've tried re-wiring, re-programming, resetting, and nothing seems to help.

Is this ipac dead?
