Looks like a lot of replacement parts on there.  Might make it impossible to identify. (?)
Back for nostalgia, based on nostalgia.

I don't know what cabinet it came from, but that door has twin Coin Mech brand single entry front plate assemblies on it.

I would hazzard a guess that this door was manufactured around 1984.
