That looks just like the trackball I got from Ultimarc, the pinouts are here:

I've tried every wiring combination and I can't get it to work.
When I use the wiring suggested by Ultimarc, the cursor will move very very slowly.  Plus it doesn't matter which way you spin the ball.  The cursor moves in only 1 direction regardless of the direction you are spinning.  I'm thinking I might have an active high/low problem or maybe there is a compatibility problem with my Belkin USB mouse hack.
The Wico trackball is made by Suzo.

Any ideas?

My first thought is that by "trying every combination" you could have fried something.

But we're only talking 5 volts here... maybe you didn't fry anything. I know that my Wico would not work until I set the correct jumper for the "active hi/low" on the optipac. I don't know exactly how you do that with a mouse hack.......

I just checked my optipac and it is infact set to active HI.

Also, I checked my wiring and one of the two middle pins is the +5v and GRND. The two exterior pins are the direction signal.

NO MORE!!

I just played Missile Command and I guess you get what you pay for.  The Wico (Suzo) trackball plays OK, but my Happ trackball is much better.  The Wico ball tends to lift up when you roll very fast which causes it to track poorly.   It helps if you push down on the ball as you move.  Also  the trackball doesn't track even unless it is level.  When you tilt the trackball, it moves faster in one direction.  This makes it pretty much useless for my new CP which has a 10 degree angle.  The overall quality of the parts seems very cheap too.

Another sucker. ;-)
Don't ever buy from Wico.
Their old 80's parts are great. The current Wico sucks.
NO MORE!!
