Four Player button issues

I helped a friend build a four player CP.  He doesn't have much knowledge of the whole thing, and I only have a two player cabinet, so we have some questions.  Why does a certain combination of button pushing lock up the machine?  Why does the third and fourth players start buttons not always work.  I noticed problems with this in TMNT and Wrestlemania.  Any help would be greatly appreciated as this is extremely frustrating!

Have you checked the wiring ? What kind of encoder ? What combination of buttons causes the lock-up ?

Sorry to leave the problem so open ended.  I am using the standard mame setup with the ipac 4.  Has anyone experienced any of these issues with four player games?  Another buddy had the same issues so he remapped everything, and I do not want to do that.  The wiring is fine.  I am using mamewah with mame.94.

I would remap some of the standard controls.  on mine Ctrl-Alt-Enter would make my computer lock up.

Did you check to see if Ctrl-Alt-Enter is a likely cause of your lockups ?

I also have a 4 Player control panel with an IPAC 4 and I learned very quickly to remap the buttons. I tried to make most buttons a letter or number and tried not to use buttons like Alt, Ctrl, etc.

I just thought maybe there was another explanation for the situation.  Remapping is an obvious fix, I just didn't think it was the only one.  As for the pixie dust, I'll take some just in case. :)
