Probably a modified Dunk Shot or Atari Football. I have a cabinet like that available in St. Louis if anyone is interested.

I'm sure Golden Tee did come in a cocktail. Because the last board I had the dip switches were snapped off and it was stuck in cocktail mode. For the cocktail mode the trackball is rotated 45 degrees.

It does indeed support cocktail mode, but it may not have actually came in cocktail format. Most manufacturers have always tried to make their games support cocktail mode because it adds little to the development time for most games, and can mean a few, or a few hundred extra sales.

Gatorcade is making one in a suitcase/box that you can plug into a TV now.

I'm trying to fit one into an old Atari cocktail using a 2 1/4" trackball.  Somebody totally demolished this old centipede cocktail so that it really wouldn't pay to convert it back.
