If I set up a cab with a monitor that requires no isolation transformer, and everything else is MAME stuff (PC mobo, with PC power supply), should I be installing any fuse lines anywhere like arcade cabinets usually have?

It's a matter of choice.

I would at least be installing one for the monitor, the PC will be fine through it's fuse in the power supply that's in the case.

I have fuses in the mains plugs for everything (flourescent tube, monitor, PC).  These are plugged into a 4-way mains adaptor which is fused, and I have a fuse in the mains plug where the machine is plugged into the wall.

I don't know if US mains plugs are usually fused, but all UK ones are...
