7 button/player template

Ok, so I've kinda got worked out what I'm going to do for my new build and have ordered a chunk of what I need.  The big question I've got is that once upon a time I got a template from here that was for 1 stick and 7 buttons (3 on top row, 4 on bottom with the leading (7th) button lower than the other 3).  I've looked around, and perhaps I'm blind but I don't see it now.  Anyone out there able to point me in the right direction?
