Midway Mfg Co. Label

I'm in the process of building a replica Mortal Kombat II cabinet and I'm having trouble finding the the light grey Midway Mfg Co. label that goes on the back of the cabinet. If anyone has a high quality scan, photo, or vector image of this label and would like to share it, I would be very thankful.

Close but it doesn't include the Bally company, just midway. I may be able to edit that label in Photoshop if no one else has a copy of one. Thanks for the quick response!
