Trouble configuring an old slikstik controller

I know this site is more about building your own arcade from scratch, so if I'm in the wrong section or the wrong forum, please let me know.
 
I purchased a cabinet with an old slikstik controller.  I believe it is a classic 2 player controller, probably built before 2004.  I followed some basic directions for installing command line Mame and Mala.  I didn't realize at the time that you have to give command line directions in the frontend for using a particular controller, so I was confused when the slikstik buttons corresponded to the wrong keyboard keys.  At that point, I probably made the wrong decision of trying to program the board with Winipac.  I think the board was probably configured correctly to begin with, but now I'm a little lost.  Because it's a pre-2004 board, I believe I have to use the old legacy version of winipac to program it.  But the legacy version of that program only seems to program coin, start, the joystick and buttons 1-8. 

I can't figure out how to program the black pause button in the upper left corner.

And there were some directions with the cabinet which said to press the pause button and the one player start button at the same time to exit the mame game or exit the frontend.  That worked in the beginning, but now that I've reprogrammed it several times, that no longer works and I don't really understand which buttons you are supposed to push, or are supposed to program to push, in order to escape?

Is there a way to restore a controller back to it's original configuration?  Or, if not, is there a way to program all the extra buttons on a slikstick classic controller using the legacy version of Winipac (or is there another program I can use to reprogram it)?
