Control Panel Input Issues - Help Please

I built an arcade cabinet a while back and have encountered some troubles.  The control panel is a 4-player style with an i-pac that then runs to the PC.  When I type on the keyboard the key strokes are read correctly however when controls on the control panel are pressed they are not read properly if at all.  Does this sound like the i-pac is in need of replacing or something else?  I am not sure how best to troubleshoot this issue.

You'll need to specify and some pics wouldn't help. Is it a USB ipac? Does it work when connected to a different computer? Is the green light lit? Does pressing buttons produce any input at all? Are you using a keyboard pass through? Have you tried removing all the buttons and wiring just 1 button to see if it works?

PS/2 4-player iPac.
Most of the buttons/joystick directions produce input as can be seen in Notepad, they are not all the correct input though.
Yes, using keyboard pass-through (keyboard plugged into iPac, iPac plugged into computer's PS/2 port).  Keyboard works correctly.
I do not see a green light on the iPac.

Two other things differ from the pic.  1)  the four empty locations bottom center are wired up, these are the Enter, Tab, Esc, and Pause.  2)  The jumpers are offset, that is to say the one on the left in the pic is on the lower two pins and the one on the right in the pic is on the upper two pins.

Also of note is that I am not using a custom key map, it it using the standard out of the box MAME configuration.

I'd go ahead and setup a keymap and upload it to the IPAC using they're utility. There's no telling what's on there now or how things are mapped.

One question though. When you press buttons, you say you don't get the right keystrokes and you can see that in Notepad. Ok. BUT. Do you get the SAME keystrokes each time you press a specific button?

If so, It would sound like the ipac is working, it just has some other keymap loaded on it.

I got it fixed.  Thank you both for your input, it helped point me in the right direction and allowed me to fix the issue.
