Issue with Opti-pac mouse button inputs

I have an "issue" with the mouse button functionality of my Opti-pac.

First the setup:

AMD XP1800 with 512 MB of DDR.
Windows 98SE.
MAME AMD K6 DOS version from several versions ago compiled to remove the "OK" screens.
Opti-pac with USB.
DOS USB is enabled in the BIOS.

Now to the issue.  The mouse button functionality of the Opti-pac works perfectly in Windows.  Every time I click one of the two buttons on my control panel allocated to the mouse buttons, they respond.  

However, when I run MAME and get into a game, I can click those same buttons, and they will only register a click about 60% of the time.  As you might imagine, that hampers one's score quite a bit.   ;)  The odd thing is, I would expect the trackball to have the same problems if it was a DOS USB issue, but the trackball works flawlessly.

Has anybody else experienced this?  I wanted to see if there was a work around other than upgrading to the Windows command line version of MAME, but if not, that's what I'll do.

<edit>  I have now tried with the Windows command line version of MAME and have the same issue with that.  So the DOS theory is out.
