Newbie pc to arcade monitor hookup

I am converting my first cabintet (Golden Tee 97) to a mame.  I have read many posts and have some clue(rally not much) of what i think i need.  I ned the following:
1. identify monitor type ( 25 inch Zenith sticker o tube says G-A63ADG25X  also has 04RC0105-005
     chassis, i believe is Wells Gardner # 518961 manufac may 1990 a plate says 25x 3219-001C  sticker on bottom says 69x2084-100    It also has 2 wires going up to cup
2. I need to hook up cable from pc to monitor, the connection to monitor has 6 spots R,g,b, Grnd, +V, +h (although only 4 wires used.  The black wire is cut and not used at all.)  other clip has onlly one wire (white, which i believe is hooked up to -H sync, does that seem right?
3.  Am i making my own wire vga to connect up to these ends?
4.  Can i use soft 15 or ultimarc vga card?
5.  When monitor was hooked up to original game, it worked fine, but after 10 or 15 min of play the game would restart after going black(seemed more like a short or something, not sure though, any thoughts.
I have included pics, sorry if not posting all this right.  1st time and would appreciate any helps suggestions on this post and others.  Thaks
