That transformer is from a Mylstar/Gottlieb "Three Stooges" game.
Clean the dust off the other side with a paint brush so you can see the numbers and the wire colors.
The monitor output is from three terminals as you can select 100 volt or 120 volt output to your particular monitor:
1 - Common (neutral) Gray-Brown-Brown
2 - 100 volts (if you're using a 100 volt monitor) This was not connected in the Three Stooges game.
3 - 120 volts (hot) Gray-Black-Black
Terminals 4, 5, 6, 7, 8, 9, 10, 11, and 12 are not used in your application
The 120 volt AC power input to the transformer does not show any numbers on the schematic. The two AC input terminals have Purple-Purple-Purple and Purple-Gray-Gray wires.

The basic wire is white insulation with three different colors of stripes.
