Street Fighter 4 button problems?

So I have my PC connected to my candy cab via the J-PAC and it's working great except I just found out that buttons 5 and 6 will not work together on the player 2-side. Discovered this while trying to do Chun-Li's ultra. Tested each button in WinIPAC and it shows as registering. Went into SF4 and tested EX moves. Weird thing is buttons 4 and 5 work together, so does 4 and 6, but just not 5 and 6?

Any ideas what could be wrong? I won't really have time to troubleshoot till later this weekend, but thought I'd ask to get some ideas on what I could check out when I do get a chance.
