Please help ID a 25" monitor

I just brought this monitor home. I know it will need a vga to cga adaptor and an isolation transformer. Can someone help me ID this thing so I know the correct parts to order:

Thanks very much. It works just fine so no cap kit needed yet (knock on wood). Can you recommend an isolation transformer? This is my first "standard" monitor so I have a lot of reading to do...

I have an iso transformer in an Arch Rivals cab I just picked up, but I don't know if they are universal. New ones don't seem to cost very much (under $25.00) so I'll buy one from Happ if someone can tell which one I need.

I found a GeForce 4 MX440 8x 64mb etc. etc. card in my garage today. I'm going to try out Soft 15KHz program with it to test out my monitor. I can't seem to find any spec sheets on this monitor. Does anyone know what native KHz it  s'posed to run? 15? 25? 31?

Am I asking the wrong question? If I just install the software will it set itself up automatically?
