Looks like you are using an ArcadeVGA with a VGA only monitor.
The card can display VGA, though you may have to use the utility from Ultimarc to enable it, as by default it is designed to output 15Khz native arcade signals, which would produce a double image on your 31Khz monitor.
EDIT: Ok, I'm out of date, the AVGA 2 card is also designed for VGA monitors.
Two options:
1) Use the DVI port. (Use a DVI-VGA adapter if you need a VGA connection). This will work like a normal gfx card
2) Use the VGA port, designed for 15Khz output, and run the Tri-Sync utility from Ultimarc to enable 31Khz output on the VGA port.
If you are only using that monitor, which is VGA only, I would just use the DVI port.
