How to fix pincushion problems?

The link is to a picture of my wells 27inch standard res monitor. It is a current production model.  As you can see it has a pretty bad pincushion problem.  I have messed with the pincushion controls on the board and this is as good as it gets.  I cant seem to get the little dip out on near the top corners.  My quistion is, what are the first steps I need to take to fix this?  What are the main causes of pincushion problems. Also my monitor gets a  fuzzy out of focus picture after about 10 minutes of being on, would these problems be linked in any way? Thanks for all of the wonderfull help in advance.

could my arcade vga card be the problem?

How old is this monitor? It sure looks like it could use a capkit.

Probably has been 6 months (suppose to be... bought it used off ebay) since a capkit.  could that cause the blurryness and the pincushion?
