Might be a bit late and you have it sorted by now but if not.

I know its an obvious one but have you loaded the AVGA drivers?.

My first thought when I read your post was that I would definitely check your AGP slot on your mobo.
It may be that this is a very old mobo and there is not enough voltage going through your AGP slot to power the card if it is a very early AGP board. Check your Bios and see what voltage is being pushed through AGP.

Another consideration is to check the card is seated properly and there is always the possibility that you have got yourself a faulty card.

I didn't touch it....honest!

Lesson on agp slots.

The AGP 1.0 specification defined 1x and 2x speeds with the 3.3v keyed connector.
The AGP 2.0 specification defined 1x, 2x and 4x speeds with the 3.3v, or 1.5v keyed connector or a 'Universal' connector which supported both card types.
The AGP Pro specification defined 1x, 2x and 4x speeds with the 3.3v, or 1.5v keyed connector or a 'Universal' connector which supported both card types.
The AGP 3.0 specification defined 1x, 2x, 4x and 8x speeds with the 1.5v keyed connector or a 1.5v AGP Pro connector.
Each up-grade is a supper-set of the 1x mode, so 4x will also support 1x. The base clock rate is 66MHz, but to achieve to 2x, 4x, and 8x speeds the clock is doubled each time. AGP uses both edges of the clock to transfer data.

AGP (1x): 66MHz clock, 8 bytes/clock, Bandwidth: 266MB/s [3.3V or 1.5V signal swing]
AGP 2x: 133MHz clock, 8 bytes/clock, Bandwidth: 533MB/s [3.3V or 1.5V signal swing]
AGP 4x: 266MHz clock, 16 bytes/clock, Bandwidth: 1066MB/s [1.5V signal swing]
AGP 8x: 533MHz clock, 32 bytes/clock, Bandwidth: 2.1GB/s [0.8V signal swing]

Now alot of older chipset motherboards only support up to agp 2x.
Some actually say 4X

No problem right???

WRONG.

I've ran into quite a few 4x motherboards that don't properly support the 1.5v siginaling. They just default to 3.3 v.

Here's a ati chart of the different agp specs for ati cards.

http://www.ati.com/support/faq/agpchart.html

Now notice that even though the chipset might support 3.3v mode that the cards themselves don't have to implement that feature.

Newer video cards only backstep to 4X, and most  of the newer cards that are 1.5volt or .8v only. The rash of problems that people are having with avga 9200 cards seem to be when they are trying to use them with older boards.

Either upgrade motherboards or find a older series 7000 based avga card.

Later,

I (and a few others) have had the exact same problem . In the end I had to buy a new motherboard as it just wouldn't work in my Dell Dimension 8100 or IBM Netvista . Sorry to be the bearer of bad news but it sounds as if you'll need to do the same .
