I figured out my solution last night... just look at MAME and let it tell me the keycodes for those elusive keys.

It was as complex as keycode_up.

As I said last night, it was major brain fade.

I too am having the same issues, trying to map keys to left ctrl, left alt etc. Can you post your pt config file.
