Wiring a monitor to a isolation transformer

I have a question about wiring a monitor to an isolation transformer.

Can anyone help me out with a diagram describing which wires from the mains should be connected to which points on the isolation transformer and then which wires from the transformer should go to the monitor?

Also, if I was to put a fuse in there somewhere, whereabout should it go?

Bob Roberts already has an excellent basic cabinet wiring diagram on his site.
