Kortek KT-1420A video Sync issue?

So... I have a Kortek KT-1420A out of a Stargate machine that died unexpectedly.  There was no high power at all and I had to replace the HOT and the Flyback as it did have a hole and arcing to the cage.  I also checked resistor marked as 509 and it measured good.  I replaced the fuse and the monitor powered up and stayed on.

Also, I have done a complete CAP kit on this.

Now the issue:

It appears that that after all this now I am not getting video sync as the game is up and running but looks like H and V sync are all over the place.  I have looked and have not found much on the web as to next steps, hence my post here to the masses.

I will attach a photo of what I am seeing on the monitor if it helps.

Where do I start this process of finding what appears to be a sync issue?

Did you buy the correct flyback?

The picture looks out of focus. Did you adjust the focus control on the flyback?

This chassis should have two slide switches for selecting positive or negative sync signals. By chance did they accidently get moved to the wrong settings when you had the chassis out for service?
