Cant get correct display on CRT

A buddy and are attempting to turn a Police Trainer 2 game into a mame cabinet. This is our first attempt so sorry if this is a noob question.  We have run into an issue getting the PC to display on the monitor. We have the ArcadeVga card but no matter what settings we choose we cannot get the monitor to display properly. At worst it’s just squiggles, at best it’s a triple image. What are we missing?

Isn't Police Trainer 2 a VGA based game?  If yes you should not be using an ArcadeVGA as that is for Standard resolution games.

If that is true, we really are a couple of fools. The whole reason we got the arcadevga card was because we could not get the original video card to display properly on the CRT monitor either.
