If you're using XP, you should try loading Joytokey as a service. You can use XYNTService to accomplish this.
I felt you pain , i had to the same problem in the past, they way i solve'it was to create a batch file to load joy to key first then the front end, then use batch to exe converter and use that in instantsheller to start my cab.
Download it from here. I set it up with AtomicFe to give you an idea.
Over here, Instant Sheller seems to defeat joytokey/not allow it to load.Edit: Instant Sheller, per se, wasn't the issue here. It was that Explorer wasn't active, hence no applications could load via 'start-up', etc.
