Mame not saving my video settings?

On my cocktail cab, I have manually changed the video mode (thru TAB) to be cocktail mode for all horizontal games I've run across.  Sometimes I come back to a game later and the setting has reverted back to standard mode.  I can't figure out why it does for some and not for others  but it is annoying. I thought Mame saved video settings into the .ini file each time a game is exited?

Update - First of all I'm using Mala.

When I change a game to cocktail mode it stays that way the next time I launch that game, but all subsequent launches revert back to standard mode. 

I've got to be missing something simple

It must be somenthing with mala, as if i remember it has an outorotate mame function, try outside mame to see if it happens outside mala.

I started mame only, and got the same results.

I change video settings to cocktail, exit the game.  Restart mame, load same game and it's still in cocktail mode.  I exit again, restart the same game, and it's back to standard mode.

Running mame .140 compiled with hi score support and no nag.  Why would it work the next time and not the next next time?
