LED Blinky Help

All I want to do is have LED Blinky light the active controls for whichever MAME game I am in. I built my cabinet a little over a year ago and have not been able to get it to work yet.
I took some screenshots trying to set it up in hopes that I am doing something so obviously wrong that it can be pointed out. http://imgur.com/a/ZVNne In the first image, whichever port is highlighted is lighting up on my control panel.

Turn on the Debug Log and Verbose options (on the Misc Options tab), run your FE and some games, then email me the debug.zip file in the LEDBlinky folder and I'll take a look.
