The + and - inputs are to let you adjust for which polarity sync signal you're using. You only use one set of inputs.
PCs conventionally output separate positive sync signals, but many of the commonly used tools to get resolutions compatible with standard res arcade monitors (like Soft15khz) change it to negative sync. I don't know off the top of my head what the ArcadeVGA defaults to.
Arcades wired for JAMMA by convention use composite (combined H/V) negative sync.
You can get something that resembles (but isn't perfectly) composite sync from separate sync signals in many, though not all, cases by just hooking the two wires up. No reason to do that, here.
So, identify what method you will use the get the PC to output a compatible signal (note: NOTHING a PC will put out by default is compatible) and what polarity sync that will result in. Choose the corresponding sync inputs.
