I had this issue, Connect to the DVI port at power on and you should see the Bios POST screen on the monitor, press F8 just after this to interupt the windows boot process and get the boot menu. Select VGA Mode from the list.
This will get you into windows on the DVI port, I found when you setup the card the DVI port no longer worked and the VGA port was always selected as a single primary display. What i did to over come this was run the TriSync application in VGA mode (As above) set the card to use DVI only (No arcade monitor) Reboot, then use the Catalyst application to setup a hotkey to cycle through Display mode and Monitors (2 seperate hotkeys) You can then use the hotkeys to switch from VGA to DVI or to use both displays.

The good news is I had the same problems (but was using Win7 and solved them by doing something i have never tried before. I read the installation instructions (and followed them word for word) on the ultimarc site. I
Most importantly - before you install the card (maybe use the MOBO vid output withnout the card installed for this bit) run this:

plug the DVI part to a PC monitor - turn on the PC and wait - you will not see a signal during POST - only when Windows is loaded.
Then install the AVGA catalyst app from the AVGA DVD.
Once installed and rebooted I connected the Arcade Monitor to the AVGA then (looking at the still connected PC LCD) I "extended windows onto the 2 monitors (clone will not work).
May have to reboot - can't remember.
Now I have the Arcade set as primary and the LCD as secondary - that way Hyperspin and MALA start on Arcade but i can actually read and manipulate the PC using the LCD. (Not sure about you but my PC desktop is hard to see on the Arcade monitor)
Once I have all the settings and stuff sorted 100% again I will remove the LCD. I have a 7" touch screen LCD i am contemplating installing that will serve the same purpose as the current 17" LCD setup monitor - some food for thought.
Why did I do this?
Well I have had an AGP AVGA card in my arcade machine for 3+ years. It was time to update to Win7 and the Pc I was using now has PCIe. - So a "start from scratch" project was started.
