They're standard game port pads hooked up with a Y adapter.

Since the copyright date on the PCB is 92-93 then the name of it is...a Gravis Gamepad! 

Seriously, the orignal gamepad (with four buttons, each a different color), was just called the "Gravis Gamepad".  Later on they brought out the "Gamepad Pro" which looked like a cheap PSX knockoff.

The original gravis gamepad was the last PC gamepad I bought (not including steering wheels)!  I remember playing commander keen with it and I even played thru doom and doom2 with that gamepad.  It's probably still in my parent's basement somewhere.

The original Gravis Gamepad is still my favorite.  I must of bought a dozen 'fancier' pads for my PC since then, but you just can't beat that gamepad.  IMO.  It just felt comfortable in my hands.
