Or was it only available as a kit? If so, what would be a good cabinet to put one in?

If you can dodge a wrench, you can dodge a cab.

Kit only.

Edit: Turns out the flyer says there was one. I only ever saw it in conversions though.

So the game was made by Technos, did Technos ever have their own cabinet for any of their games in the U.S. or were they always licensed/distributed by other companies such as Taito or Data East?

If I remember right, the SDB I played as a kid was in a Nintendo cabinet, but I think something along the lines of a Data East cabinet (e.g., Karate Champ style) would be more fitting. That way I could use the SDB marquee and bezel as-is, whereas with a Nintendo cabinet, there is no provision for installing a cardboard bezel, and the marquee area is skinny -- not to mention all of the other nonstandard features of Nintendo cabinets.

Also, was there Super Dodge Ball sideart?

Apparently there is a dedicated Super Dodge Ball machine. What kind of cabinet is that? Something made by The Leland Corporation? A generic?

Leland used that same cab for a couple other games. Take a look.

For some reason all of their cabinet designs are a bit odd. Here's some more funky examples:

Quarterback, Strike Zone, and Viper
Attaxx
Brute Force
Brevity is not my strong suit.
