I'll clarify some points as much as I know:
1. Crt emudriver is installed and has always been working fine at 15khz.
2. I have an HD7750 firmwared to output 15khz.
3. dipswitches and set up wise, everything works perfectly without the amplifier (even after all testing yesterday, which is good
4. According to the new vga pinout (thanks!), I wired ground pin 5 to GND in the amplifier. Nothing changed but now at least I'm sure its the correct pin

- I removed the VGA diagram in the post too.
5. as I already had cut the trace on the amplifier yesterday as I thought this might be the issue, I guess I have no way to undo it and try again with composite sync...for now its separated wiring H and V. Not sure maybe I need to solder the parts again with a wire - I am really new on that, or order another amp and retry.
Now I hesitate if its worth as like you said if the VGA DSUB already takes the VGA levels fine, maybe i wouldn't see any difference at all. I didnt think of that before, but maybe with the VGA itself and the JPAC there is no further need of amplifier?
Anyhow, I also emailed Andy to see if that's the case, probably he also knows if indeed I could gain some image improvement or not
