MAME Key Assignments fall out

I’ve got the most recent version of MAME. Every now and then my generally assigned key assignments will fall out. I can reassign and all is well, but does anyone know why this might happen? Could be coin, player start or other buttons.

Are you using MAME alone or with a Front-End(Launcher)?
