Big box incorrectly shows game over screen

I’m having a strange focus or splash screen problem with big box and Mame

Sometimes I’ll see the big box ‘game over’ screen even though the game hasn’t ended. I can hear the game playing in the background and can alt tab back to it.  I’m not sure what is triggering big box to display the game over screen and force focus away from the game.

It’s very annoying for sure.

Any ideas?
