A few Maximus issues

I'm having some trouble using my MAME cabinet with MA.  I am using an X-Arcade dual stick.

1.  I can't get L R to work for "ok" when starting a rom.  (It doesn't work in just MAME either, but left and right work within the game).
2.  Maximus sometimes locks on black screen when exiting game using right pinball + player 2 start
3.  If it gets back to game select it is a smaller version of game select without the cabinet picture on the side
4.  I can't get the controller changes or delays to stick after changing them using ctl p for preferences.

Thanks for the help, the MA forums are down and I didn't see any help here after a quick search.

Thanks for the help.  I'll try these solutions and post my results.  Do you save the control delays on the control save button?  I know it sounds like a dumb question, but I don't see an option to save other than at the bottom of the control1 tab.  Nothing in the delays tab.
