TV-Out, Overscan, Shift MAME slightly to the right

Anyone know of a way to force mame to display either slightly smaller then 100% of the screen (in a fullscreen mode) or to shift the display a bit? 

I'm using a TV-out and a small portion of the screen is being cutoff on the left side of the screen.  I can accomodate for it in front-ends and the like by making skins that don't use the space, but MAME does full screen so I can't really figure out how to get it working properly.

edit: Just for some more info, I'm using a PC connected to both a monitor and a TV, the TV is the primary out for the HTPC when using MAME, etc.  The TV-Out is setup via TV-Tool, and I can't seem to get AdvMame working because of it.  using the standard driver tv-out isnt an option because it's screwed up for my card (GeForce2 Pro)
