Sorry, I just got your message today.  I replied to your message.  I sold the machine, but if I remember right I used speed 20 and sensitivity 25.  Older versions of MAME used to have that as a default, but newer ones seem to have sensitivity set at 100.

I had my cab boot right into Tempest with no front end.  Most front ends allow you to choose what keys move you through the menus, so I'd use p1 start and p2 start to move up and down through the menus.  I'd then set the fire button to start the selected game, and finally p1 and p2 start at the same time to exit a game.
