I think you have to turn on mouse support, and then multi-mouse support. As each opti-pac is a different mouse technically, you need to tell MAME to treat them as individual devices. I really don't know how to do that in that old version of MAME though.
There is some info here I googled up: http://forum.arcadecontrols.com/index.php?topic=51327.0
But you seem to be using a MAME version even before this. (.104 I think in the above thread?)

Multimouse is not an option in MAME v0.92.
- If you don't enable multimouse, two wheels add together to control the Y-axis.
- Upgrade to v0.104u5 (u5=update 5) or newer -- preferrably the newest that you find a ROMset for and that your CPU can run full speed.
With your upgraded MAME, you'll need to generate a mame.ini file using the "-cc" (create config) software switch one time. i.e. Run "mame32.exe -cc" from command line.
- If you do this more than once, it will create a default mame.ini that overwrites any changes you make in the next step.
Edit mame.ini with Notepad.
- Enable both mouse and multimouse -- 0 = disabled, 1 = enabled. Save and exit.
--- Code: ---#
# CORE INPUT OPTIONS
#
coin_lockout 1
ctrlr
mouse 1
joystick 1
lightgun 0
multikeyboard 0
multimouse 1
--- End code ---
Launch MAME, map the wheels, and play some games.

Win7 supports all latest mame, there's no need for Win10. XP is only supported to ~mame 0.180, though there are modded versions available. Mame comes with its own built-in UI these days.
You should consider Groovymame, is basically just a better version of MAME, especially for CRTs, and makes setting up all the per-game video options much easier (mostly it just works and you just don't have to worry about it).
