Do the turbo twist spinners fit in the same hole as the regular buttons do?

Regular TT2 - Yes.  Randy designed them to do just that.  You can tighten the nut using a standard button wrench. (right)
   

TT2 High/Low - No.  Different mounting method.

Just curious.  What is the mounting method for the push/pull spinner?  I checked Randy's site and didn't see any mention of this.

how about the ultimarc spintrak? same as TT2 regular?  which one is better?

They are fairly similar.  Small difference in resolution IIRC, different knob choices.

Looks like when the flywheel is installed on the SpinTrak, it prevents you using a button wrench.

To avoid throwing gas on that fire, I'll just say that after long and careful evaluation, I chose the TT2 spinner, a U-Trak trackball and a Mini-Pac Opti encoder w. wiring harness. YMMV.

Other than it being a relatively low-demand item, I'm not sure why he doesn't have the template PDF link or a pic on the product page.

It comes with a template and you make a crazy hole that looks like this:


It's not as bad as it looks but it does take a bit of finesse.
