Well, Ben Heck clips down a NES pretty small to go in his portable NES. There are whole sections of the pcb you can literally cut off IIRC. I would really like to see the innards. Why would the creator post this without the innard shot is my question.

Umm... according to the article. It was a forum member of benheck.com, not Ben himself that did that. Ben's version is a custom made controller with a built in screen.

That pic looks fake. Mainly because the cuts are so perfect. No signs of the plastic marring or shaving at the cut's edges. That's practically impossible. Cuts like that can only be done with a precise cutting machine. Plus the cartridge looks immaculate. You'd think after so much work was put into it, it would look a little deteriorated. Not one single scratch or spec of dirt. That's really hard to believe.
Of course I could be totally wrong, but hey that's just my observation.
