Bet a lot of that is fluids from his condition... water buildup is a very common symptom of pneumonia and heart problems.

...of course, what caused those issues could be a story... given that among people with the means to get medical treatment, severe pneumonia usually doesn't happen unless their immune system is somehow compromised... and given his lifestyle that type of condition would have an obvious cause.

i knew a guy who collapsed a lung from smoking too many bongs. i'm willing to bet it's complications from something along those lines and they are just calling it pneumonia...
