Can anyone give suggest a video card, software/drivers/etc and if need specific monitor for a MAME cab running pure DOS?

If you're just using a PC monitor, pretty much any card should work fine.
If you're using an arcade monitor, either get the Trident Blade T64 (amazon sells it for $27), or wait a couple weeks until Andy Warne's video card comes out, it is supposed to be the holy grail of video cards for arcade monitors (http://www.ultimarc.com). It costs more than the Trident, supposedly around $90, but Andy says it is in a completely different league than the T64. (Given Andy's involvement in this community and his great products, I would tend to believe his assertion.)
Oh, if you're using a TV with svideo, ATI cards are supposed to be very good. I think composite TV's are generally not recommended for MAME, so try to avoid those if you can.

I have already built a cab with a 17" PC monitor.
I would like to use an arcade monitor in my next cab.

I have a Wells Gardner D9200 27" Monitor, that accepts a VGA feed from my video card. I haven't had a chance to use it in a setup yet (it's still in the middle of being built), but everyone loves this monitor. I ordered mine from American Gaming and Electronics, which is a division of Wells Gardner. $466.50 after shipping. They only charged me $30 to ship via freight! What a deal! Here's a link:

That seems expensive for a monitor. What does it buy you? I bought a whole sit down san fransisco rush cabinet with a 25" screen for $500.
